Sublime Care Solutions is dedicated to partnering with commissioners, health professionals, individuals, and their families to support the Transforming Care agenda set forth by NHS England. Our mission is to enhance health and social care services, enabling more individuals with learning disabilities to live within their communities, surrounded by the right support and close to their loved ones, rather than in hospitals or secure settings.

 

We acknowledge that many individuals with learning disabilities have spent extensive periods in hospital settings, often experiencing numerous restrictions that hinder the development of their skills and independence. Moreover, there are others who remain at risk of hospital admission due to a lack of appropriate support. At Sublime Care Solutions, we firmly believe in a values-based approach, as it has the potential to profoundly improve the lives of those we serve.

 

To ensure a seamless transition to supported living, we employ a collaborative multi-agency partnership approach that places the individual at the center of their care. Our support is rooted in our core values, which prioritize social inclusion, enhanced quality of life, and discourage the use of restrictive practices. Additionally, our in-house psychology team provides proactive support to individuals and their support teams, promoting their overall wellbeing.

 

At Sublime Care Solutions, we embrace a positive risk-taking approach that empowers individuals to lead fulfilling lives. We strive to increase their independence, seeking to minimize their reliance on paid support. Drawing on our expertise and relationships with registered housing providers, we diligently source appropriate permanent housing options for those we support. These living environments are thoughtfully designed to meet individual needs and incorporate person-centered assistive technology.

 

We have made significant investments in our step-through services, which enable individuals to prepare for their permanent homes through short-term tenancies. During this transitional period, we equip them with the necessary skills to live more independently. Additionally, we offer permanent high-intensity residential care and supported living services in Leicester city and Leicestershire.

 

To ensure the highest standards of care, we prioritize staff training in PROACT-SCIPr-UK®, positive behavior support, and trauma-informed care delivery. All training initiatives are tailored to meet the unique needs of each individual we support. Furthermore, we involve the individuals themselves in the selection of their support team, ensuring that staff members are well-matched to their specific requirements.

 

We are committed to delivering exceptional care and support in line with the Transforming Care agenda. Our high intensity residential care and supported living services cater specifically to individuals within this pathway or those with complex care and support needs that demand robust accommodation.
