Our approach is rooted in evidence-based, outcome-focused assessments to ensure our support empowers individuals to lead the lives they desire. We adopt Person-Centred Approaches to ensure that the individuals we support are central to every decision affecting their lives.

Person-centered support entails constant listening, learning, and understanding what matters to a person, both now and in the future. It encompasses consistent communication with those we support and those who know them best. At Sublime Care Solution, practicing a Person-Centred Approach involves providing individuals with opportunities to achieve their goals within the context of the service they are receiving.

 

Co-Production

At Sublime Care Solution, we believe in the importance of involving the individuals we support in all our processes.

We understand the importance of their input in decision-making processes concerning their support, staffing changes, or the introduction of new activities. Our Person-Centred Approach ensures we truly comprehend their goals, preferences, hobbies, and interests. We actively involve the individuals we support, valuing their input in various ways.

 

Positive Behaviour Support

Our Positive Behaviour Support (PBS) Framework at Sublime Care Solution is guided by the principle of establishing trust and rapport with the individual, their family, and their support team. Our organisation's DNA, our values in practice, forms a key part of this framework and significantly enhances the well-being of individuals, reducing their need to communicate using behaviours of concern.

 

Upon establishing trust, we collaborate with the individual and their support network to understand what a behaviour of concern communicates, and coach the team to provide support in the most person-centred and least restrictive manner possible. Our proactive approach, rather than crisis-driven, focuses on the long-term support of each individual to live a life that is meaningful to them and to communicate their needs safely.

PBS at Sublime Care Solution is not a one-size-fits-all approach. Our actions are dependent on the individual's needs and preferences, considering the people and the environment surrounding them. We leverage our wide range of expertise and resources, including partnerships with Redstone PBS, families, and experienced experts. By collaborating with local professionals, we find the most suitable path forward for each individual.

Sublime Care Solution's approach enables support in various settings such as their homes, the community, residential care, and a wide variety of supported living arrangements.

We offer a spectrum of support for children and young people, ranging from high-quality, local Ofsted-registered supported accommodation to highly specialized residential care. Our first children residential home project in Leicestershire is set to open soon.
