Sublime Care Solution is steadfastly devoted to assisting individuals transition from long-term hospital stays, inpatient facilities, and assessment and treatment units back into their own communities. This initiative aligns with the UK's National Plan titled the Transforming Care Programme (TCP), which promotes the philosophy of 'home over hospital.' TCP aims to aid people with learning disabilities who may also grapple with mental illness, behavioural issues, significant sensory problems, or forensic complications associated with their learning disabilities. We provide a path away from secure hospital settings, towards a more autonomous and fulfilling life.

 

Our exclusive service, "Transforming Care (Home Over Hospital)," includes specialised supported living services and solo-occupancy residential care homes. This is an initiative dedicated to empowering these individuals to lead fulfilling lives outside of hospital settings. 

 

Our strategy in aiding individuals during their transition from hospital to community living is person-centred, result-oriented, aligned with government guidelines, and a reflection of best practice.
